INSTRUCTIONS FOR CLEARANCES & FORMS

 All applicants, employees, and independent contractors are responsible for all costs associated with obtaining Clearances.
 

1. Act 34 State Police Criminal Record Check - 1-888-783-7972

a. Go to www.psp.pa.gov
b. Click on Request a Criminal History Record
c. Click on Pennsylvania Access to Criminal History (PATCH) Website under # 1
d. Click on submit a New Record Check, purpose of the clearance is Employment
e. There is a $22.00 fee
f. Enter your information, print the Certification Form, and bring a copy to the Human Resources Office
 

2. Act 151 Child Abuse History Clearance  - 1-877-343-0494

a. Visit www.compass.state.pa.us/cwis/public/home(Child Welfare Portal) click on Create Individual Account and enter the information to create your Individual Login. Login and follow the instructions. For “Purpose of Clearance”, select School Employee Governed by the Public School Code.
b. There is a fee of $13.00
c. Applicants will receive their results more quickly via email and will be able to print their results from any computer. Once completed bring a copy to the Human Resources Office.
 

3. Act 114 Federal Criminal History Record Check - 1-844-321-2124

a. Applicants are required to pre-register before being fingerprinted
c. Select the “Digital Fingerprinting” logo
d. Enter the Service Code 1KG6XN (School Districts)
e. Click on Schedule or Manage Appointment; complete the fields and click on Next until all registration pages are complete.
f. When registering you have the option to schedule an appointment to have your fingerprints taken. Appointments are not required but recommended.
g. Payment is due at the time of fingerprinting, $25.25, which can be paid by money order, cashier’s check, or major credit cards.
h. Once fingerprinted, bring the IdentoGo Receipt to the Human Resources Office.
 
Note: You will be able to track the status of your fingerprints at the IdentoGo website and use the IdentoGo Receipt as verification you applied for your FBI clearance.
 
Clearances must be renewed every five years from their issue date.
 
It is the responsibility of the employee to ensure that his/her clearances are current.
 
4. Act 24 Arrest/Conviction Report and Certification Form 
 
Act 24 of 2011 contains a number of significant changes to the PA Public School Code designed to enhance the safety of school children.
 

The law includes an important mechanism to help ensure current school employees, who may not have been subject to a previous background check, are now required to provide assurances they have not been previously arrested or convicted of Section 111(e) offense.  Accordingly, under Act 24, all current school employees are required to complete and return to a designated School Administrator (Superintendent or his/her designee) a form developed by PDE to report prior arrests or convictions for any offense listed in Section 111(e).  This form (PDE-6004) is available from the following link:

A hard copy of this form will also be available for you at your work location and the District Administration Building

ALL SCSD Employees and Independent Contractors and their employees are required to do the following:
  1. Complete and sign form PDE-6004
  2. Return the form to the Business Office, 1100 E. Main Street, Roaring Spring, PA 16673
  3. If an employee refuses to submit the form, Act 24 allows for a school administrator to require that the employee submit to a current background check under Section 111(e).
  4. In addition, school employees and independent contractors and their employees will be required to report to the School Administrator within seventy-two (72) hours any arrest or conviction of an offense listed in Section 111(e) that occurs after September 28, 2011.  The PDE form should be used to report these arrests or convictions to the School Administrator.  Failure to make timely notification of any arrest or conviction of an offense listed in Section 111(e) that occurs after September 28, 2011, is subject to disciplinary action up to and including termination.
5. Act 168 Sexual Misconduct/Abuse Disclosure Release Form
 
Applicant must complete this form for the current employer and must also complete one form for each of the applicant's former employers that were school entities or where the applicant was employed in a position having direct contact with children.  (Therefore, the applicant may have to complete more than one form.)
